WholeCare’s Post acute care services are offered to those who have suffered strokes, heart attacks, pneumonia, broken bones, and other unexpected health events. In-home recovery with a caregiver has been proven to prevent readmission, speed up recovery, and cost less. Post acute care is laser-focused on offering you unwavering support to help you maintain the level of independence and physical mobility you gained in the rehabilitative and hospital space.

These diagnoses are often accompanied by fear, uncertainty, and more questions than answers. A significant concern many families have centers around staying home as the disease progresses. End Of Life CARE
